On June 10, 2015, Assurant Health’s parent company, Assurant, Inc., announced an exit from the health insurance marketplace to focus on housing and lifestyle specialty protection products and services. Assurant is winding down its major medical operations. Assurant is committed to providing a smooth process for our customers and will meet all claims and benefit commitments.

Assurant Health no longer offers new sales of individual major medical plans, short term medical and employer fully insured plans.

Q: How do I get a claim form for my prescriptions?

The pharmacy typically submits your prescription claims electronically, however if you purchased a prescription, and the pharmacy could not submit the prescription electronically, please fill out a prescription drug claim form, and mail it to the address located on the form. Q: Where do I send claims?

Refer to the back of your health insurance ID card for claims submission information.

Q: Can I fax in a claim?

Yes. Our fax number is (608) 741-4989.

Q: How long does it take to process a claim?

The amount of time it takes to process a claim depends on the information submitted. In general, you should receive an Explanation of Benefits within 3-4 weeks. If additional information is required to process a claim, we will notify you, and the claim could take longer to process.

Q: How do I appeal a claim denial?

If you believe your claim has been processed incorrectly, please contact our Customer Services Department. If you do not agree with the denial of a claim, please send an appeal in writing to Assurant Health, Correspondence Department, P.O. Box 624, Milwaukee, WI, 53201-0624. Note any extenuating details, include any documentation pertaining to the appeal, and keep a copy for your records.

Prescription claims may be submitted by your pharmacy. However, if you paid for the prescription without your insurance card, you may submit a claim form PDF Icon.

Electronic Claims Submission

Assurant Health contracts with a number of primary networks that can be identified by the appearance of the network's logo in the lower left-hand corner on the left-half of a member's medical insurance ID card. Please be sure to review the back of that ID card for the most current claim submission process.
