Individual Conversion Plan

Eligibility
Our individual conversion plans, CoreMed^SM, OneDeductible and SaveRight^SM, are available to customers whose Assurant Health insurance coverage is ending.

To determine your eligibility for one of our conversion plans, you may contact the Department of Insurance (DOI) at 800.927.4357 or the Department of Managed Health Care (DMHC) at 888.466.2219, or visit the DMHC Web site at www.dmhc.ca.gov and search for “individual conversion plans.”
