This price guarantee is good for services received up to 30 days after the date Assurant Price Check provides pricing.*
We guarantee you will not be responsible for costs exceeding the quoted price range, provided you receive services for the procedure code shown, from the doctor listed. It’s important to know:
- Your doctor may bill you for services beyond what you searched for.
- This guarantee does not apply to different or additional services billed by your doctor, or for services billed by a different doctor.
- Doctors at the same location may charge different rates.
The guarantee applies only to medical services for which First Health Network discounts apply. Be certain the services you receive are eligible for discounts by making sure:
- You have an active Assurant Health Access policy when you receive services.
- Services are not excluded or subject to the pre-existing condition limitation under your plan.
- Your doctor is part of First Health Network when you receive services. Doctors may have been added or deleted since this online directory was last updated. That’s why it’s important to confirm your doctor is part of the network by calling either the doctor’s office or First Health at 888-367-3102 before your appointment.
- You present your ID card every time you see a network doctor and “assign benefits” to him or her by requesting the forms that allow us to pay your doctor directly.
Assurant Price Check is not a guarantee of Assurant Health Access plan benefits. Please refer to your plan for specific information on all terms, conditions, exclusions and limitations.
