2Coverage is renewable to age 75 in most states provided you have not moved to a state where we do not offer this plan or no longer qualify as a dependent. Coverage is renewable up to age 70 in CO, DC, ID, IN, KY, MA, MD, MS, NC, NH, OH, OR, SC, SD and UT. The Company has the right to change premium rates upon providing appropriate notice.
