How it works

The Basics
When you apply, you receive your own personal Assurant Health ID card. It gives you access to quality health care when you need it, for the medical services you use most – like doctor office visits and prescription drugs. And there are also benefits for services that can help keep you well. 

You can use any doctor you want. Choose one in Assurant Health’s network and you may be able to save even more. Even when you reach your benefit limits, you'll have access to network discounts.

The providers send us the bill directly. Some employers let you pay premiums through a payroll deduction plan or we can bill you directly.

Application Process and Effective Date
You’ll have the opportunity to thoroughly evaluate the three Assurant Affordable Health Access Plans, and then determine which plan best meets your needs and budget. When you sign up, the effective date (1st or 15th of the month) of your policy will be the date set by your employer for payroll deduction. You can work with an agent for other billing methods and your effective date.

The application process is simple with no medical questions for Health Access Plan A and only a few medical questions for Health Access Plans B and C. For Health Access Plan A, once you submit your Enrollment Form and select a payment method, the application is complete. You will first receive your welcome kit by mail, followed by your personalized health plan ID card. For Health Access Plans B or C, once your application is approved, you will receive your welcome kit by mail, followed by your personalized health plan ID card.

Payroll deduction - how does it work?
Automatically. If your employer chooses payroll deduction, just tell your employer that you’d like to sign up for an Assurant Affordable Health Access Plan, and have the monthly premiums automatically deducted from your paycheck. Payroll deduction is not available in all states.

You have a 10-day free look
When you receive your policy, you’ll have 10 days to review the coverage. If you decide to cancel within that time, you will get your money back, no questions asked.

One-year rate guarantee
Once you are enrolled, your coverage can’t be cancelled because of claims or a change in your health as long as you pay your premiums. And we guarantee that the rates will not change for the first 12 months that your policy is in force. For Health Access Plans B or C, your policy could be cancelled if there is misrepresentation of health history.
