Gaining greater independence after a major illness, injury or surgery can put you on the path to better health. That’s why we’ve negotiated with rehabilitation centers that offer specialized inpatient and outpatient services dedicated to improving, maintaining or restoring physical strength, cognition and mobility.

HealthSouth
Full-service HealthSouth rehabilitation centers are located in AL, AR, AZ, CA, CO, FL, GA, IL, IN, KS, KY, LA, MA, MD, ME, MO, NH, NJ, NM, NV, PA, SC, TN, TX, UT, VA and WV.

Outpatient-only HealthSouth rehabilitation centers are located in CT, DC, HI, IA, MI, MN, MS, NC, OH, OK, OR, RI, WA and WI.

Before you receive medical care or service, it’s important you contact us as a service may require a nurse review per your contract (not required for Assurant Health Access plans). Please call the customer service number, located on the back of your ID card.
