Employment Benefits

Secure both your present and future by choosing from a variety of benefits to best meet your needs and accommodate your lifestyle. You and your dependents are eligible to participate on the first day that you are an active, regular, full-time employee or an active, part-time employee scheduled to work at least 20 hours per week. Your domestic partner and/or domestic partner’s children are also eligible for medical, dental and life benefits. Please note that these benefits are subject to change and not all employees are eligible to receive all benefits.

Medical Benefits

Six options are available to you in choosing your Medical Benefits:
Plan A (in PPO area/tiered deductibles and coinsurance)
Plan B (all areas/optional PPO access and higher deductible)
Plan C (out of PPO area/Traditional Indemnity Plan)
Plan D (an HSA compliant high deductible health plan)
Health Maintenance Organization (HMO)
Waiver of Coverage Credit

Dental/Vision Benefits

  • Preventive Dental benefits paid at 100% of usual and customary charges with no deductible
  • The Dental Plan also offers coverage for basic, major and orthodontia services
  • Enrollment in the Dental Plan brings automatic membership in the Vision Service Plan (VSP). VSP offers savings up to 20% on eye exams and 20% on glasses with no deductible

Disability Benefits (Three Stages)

  • Salary Continuation (New hires have a six(6) month waiting period)
  • Short Term Disability (STD)
  • Long Term Disability (LTD)
The company pays the full cost of your STD and LTD premiums. <

Flexible Spending Accounts

Lower your taxable income while increasing your spendable pay by choosing one or both of these accounts:

  • Health Care Flexible Spending Account
  • Dependent Care Flexible Spending Account

Pre-tax funds are deposited into your FSA through payroll deductions. Elect an annual amount up to $5,000 in each account. Funds are reimbursed to you when you submit paid receipts for eligible health care and dependent care expenses.

Health Savings Account

You can participate in a Health Savings Account when you elect medical Plan D. A Health Savings Account allows you to pay for qualified medical expenses on a tax-advantaged basis.

Life, Accidental Death & Dismemberment (AD&D) and Dependent Life Benefits

Basic Life and AD&D coverages are provided at no cost; benefits are equal to one times your plan pay (calculation based on your salary and target bonus or subsidy).

Supplemental Life and AD&D benefits can equal from one to five times your plan pay; the amount and cost of your supplemental life benefits are based on your plan pay, age as of December 31 of the plan year and whether or not you are a tobacco user.

Dependent Life benefits are available at various levels of coverage.

Pension Plan

The Pension Plan is paid for by the company. The plan pays a benefit using a formula based on your years of credited service and your eligible pay. You are 100% vested after three years of vesting service. If vested, you receive the value of your benefit upon leaving the company. Eligibility for the Pension Plan requires:

  • Completion of one year of service during which you worked 1,000 hours with a participating Assurant Health company
  • That you be at least 21 years of age

Participation in the plan is automatic on either January 1 or July 1, following or coincident with your first day of eligibility.

401(k) Plan

As soon as administratively feasible after your hire date, you become eligible to participate in the 401(k) Plan. You may contribute from 1% to 50% of eligible pay to the plan on a pre-tax basis, after-tax basis or a combination of the two. You are eligible to participate in the plan if:

  • You are regularly scheduled to work 20 or more hours per week for a participating Assurant Health company
  • You are scheduled to work fewer than 20 hours per week for a participating Assurant Health company, after completing one year of service during which you worked at least 1,000 hours

You are eligible to receive company contributions after one year of vesting service and 100% vested in company contributions and earnings on company contributions after three years of vesting service. Company contributions are made on the first 5% of pay you contribute to the plan pre-tax, based on your years of service as follows:

  • If you have at least one year and less than five years of vesting service, the company will contribute 100% on the first 3% of eligible pay you contribute pre-tax and 50% on the next 2% of eligible pay you contribute pre-tax
  • If you have five or more years of vesting service, the company will contribute 200% on the first 3% of eligible pay you contribute pre-tax and 50% on the next 2% of eligible pay you contribute pre-tax

Company contributions are credited to your account as soon as administratively feasible after the end of the plan year, provided you are employed on the last workday in December.

Holidays

All regular full and part-time U.S. employees who are regularly scheduled to work at least 20 hours per week receive 9 paid core holidays each calendar year. There is also the possibility that 1 or 2 additional common holidays may be added to "bridge" a core holiday to a weekend (where the core holiday occurs on a Tuesday or Thursday).

Paid Time Off (PTO)

Assurant's Paid Time Off (PTO) policy is designed to provide regular full- and part-time employees who work a minimum of 20 hours per week with sufficient paid time off for personal enjoyment while balancing the needs of co-workers, the business and the practices of our businesses.

Paid time off is based on your years of service, job tier and benefit eligibility. Paid time off starts at a minimum of 14 days or higher depending on job level.

Paid Time Off (PTO) Buy Option

You may buy PTO at a minimum of four hours to a maximum of the number of hours in your normally scheduled work week. The cost of your bought PTO is determined by your hourly salary at the time you buy PTO, with the payroll deduction spread over 24 paychecks. Bought PTO cannot be carried over to the following year. (Taking PTO is subject to your supervisor's approval and workflow needs. You must check with your supervisor before buying PTOn.) PTO is used in the following order:

  • Carryover (up to 15 days of unused earned PTO from the preceding year; purchased PTO time cannot be carried over)
  • PTO time earned in the current year
  • Bought PTO

Continuing Development/Tuition Reimbursement

Parking and Bus Subsidy

Parking is available at a subsidized rate via payroll deduction. The company also subsidizes bus transportation by contributing toward the cost of bus passes.

Employee Assistance Program (EAP)

Confidential, professional help with personal and family concerns is available to Assurant Health employees throughout the United States, 24 hours a day, 7 days a week. Assurant Health pays for up to six sessions for you and your dependents, per calendar year. The EAP's licensed professionals will assist you in making an appointment with a local provider

Dress Code

Our dress code is business casual from Monday through Thursday and comfort casual on Friday. The dress code may vary by building, location and business need.

Environment

You will work in a healthy, safe and pleasant environment. All of our buildings are smoke-free, though we do have designated areas on our grounds for smokers. We are also committed to ensuring you a drug-free workplace and therefore require pre-employment drug and substance screening for all final employment applicants.

Employee Matching Grant Program
The Assurant Health Foundation

Provides financial support to nonprofit organizations and a matching grant program that offers one for one matching of direct employee and retiree contributions to qualifying organizations, from $20 up to $2,500 per year, per employee.

Employee Volunteer Program

Allows full-time employees up to eight hours and part-time employees up to four hours of paid volunteer time per year in the service of qualifying community organizations.
