IL Confidentiality Procedures

People experiencing actual or threatened violence frequently establish new addresses and telephone numbers to protect their health and safety.
Illinois law establishes requirements for health companies so that they effectively respond to reasonable requests for receipt of claim-related
information by alternative means and keep that information confidential.

Assurant Health has implemented a confidentiality protocol to accommodate a reasonable request to receive communications of claim-related
information by alternative means or locations if the requestor clearly states that disclosure of all or part of the information could endanger
the insured.

Assurant Health Confidentiality Protocol:

This workflow applies to all insurance plans for instances when a covered person indicates they need us to protect and privatize their records, personal
information, or whereabouts due to a domestic abuse or life endangerment situation.

Contact us using the telephone number on the back of your ID card or in writing to request any of the following:
  • To withhold information from a policyholder due to life endangerment/domestic violence threat
  • Change of address or phone number due to a life endangerment/domestic violence threat
  • To release information from an attorney by warrant or subpoena.

Submit the following request in writing. In addition, NY customers must have this request notarized.

  • To revoke a life endangerment/domestic violence related address or phone number change
