Real Choices I  — Real Choices I plans are designed for employers who want to provide extensive coverage. They include many benefit choices, enabling an employer to create a plan that satisfies the unique needs of employees.

Real Choices II — Real Choices II plans offer the most choices. The plans are designed for the employer who wants maximum plan flexibility.

Real Choices Indemnity — Indemnity plans offer employees complete freedom to choose doctors and hospitals. This plan is best suited for those who live and work outside a PPO network area.


Plan Features*

  • All Real Choices plans have first-dollar benefits for preventive medical services from a network provider. Depending on the plan selected, benefits range from $200 to $1,250 per year. The deductible and copay are waived until the benefit maximum has been reached.
  • Lifetime Maximum Choices — up to $10 million
  • Annual Deductible Choices — choose from $0 up to $10,000
  • Office Visit Copay Choices — choose from a wide variety of copay options
  • Coinsurance Choices — select the amount employees pay after their deductible is met
  • Prescription Drug Coverage — broad choice of drug copays, including a $0 copay

In addition to the extensive core benefits Assurant Health provides, there's extra coverage for your employees' unique needs.

Life and AD&D, Short Term Disability and Dental Insurance
Term life and accidental death and dismemberment insurance can be added to a group medical plan. The amount of life insurance is determined by the Real Choices plan selected. 

Disability insurance can also be added, with coverage beginning on the first day due to an accident and the eighth day due to illness. The benefit is payable for a maximum of 26 weeks and the benefit amount can be based on salary or a flat amount selected by the employer.

Dental insurance can be added to the medical plan chosen or can be purchased as a stand-alone product for groups of three or more covered persons. The dental plans include first-dollar coverage for preventive services such as cleaning, x-rays and exams.

Assurant Health's Discount Card — When you choose a group health plan from Assurant Health, you can select a discount card which provides up to a 60% savings on valuable health-related products and services (many of which are not covered by the insurance plan) at thousands of participating providers nationwide.  Discount plans are not insurance.
