Assurant is winding down its major medical operations. Assurant is committed to providing a smooth process for our customers and will meet all claims and benefit commitments.

Q: How do I get a claim form for my prescriptions?

The pharmacy typically submits your prescription claims electronically, however if you purchased a prescription, and the pharmacy could not submit the prescription electronically, please fill out a prescription drug claim form, and mail it to the address located on the form. Q: Who do I contact if I have a question?

All inquiries must be submitted in writing to PO Box 624, Milwaukee, WI 53201-0624.

Q: How do I appeal a claim?

If you believe your claim has been processed incorrectly or if you do not agree with the denial of a claim, please send an appeal in writing to Assurant Health, Correspondence Department, P.O. Box 624, Milwaukee, WI, 53201-0624. Note any extenuating details, include any documentation pertaining to the appeal, and keep a copy for your records.

Electronic Claims Submission

Assurant Health contracts with a number of primary networks that can be identified by the appearance of the network's logo in the lower left-hand corner on the left-half of a member's medical insurance ID card. Please be sure to review the back of that ID card for the most current claim submission process.
